1. Enter expense items daily
2. Enter each item in ink.
3. Retain copy for your records
4. Obtain receipts for all expense items
5. Business meals cover employee only meals (no client).
6. Itemize entertainment expenses on the reverse side of this form.
7. Include transportation expense connected with an entertainment or meal expense - see reverse side.
8. List details of other expense on the reverse side.
